NSIB Recovers Key Evidence as Probe Deepens Into Warri-Itakpe Train Derailment

The Nigerian Safety Investigation Bureau (NSIB) has intensified its investigation into the tragic derailment of a train operating on the Warri-Itakpe railway corridor, announcing that critical evidence has been recovered part of ongoing efforts to uncover the cause of the accident.

The derailment, which occurred on Monday, June 8, 2026, along the Agbor axis of Delta State, disrupted operations on one of Nigeria's most strategic standard-gauge rail lines and raised fresh concerns about rail safety across the country.

In a statement released on Wednesday, the NSIB disclosed that significant progress had been made in the investigation, with investigators successfully retrieving vital evidence and completing several key on-site assessment activities aimed at determining what led to the incident.

Leading the investigation was the Director-General of the NSIB, Captain Alex Badeh Jr., who arrived at the accident scene shortly after the bureau was notified of the derailment. Accompanying him were the Managing Director of the Nigerian Railway Corporation (NRC), Mr.

 Kayode Opeifa, and the Permanent Secretary of the Federal Ministry of Transportation, Engr. Funsho Adebiyi, highlighting the coordinated response by relevant government agencies.

According to the bureau, investigators carried out extensive documentation of the accident scene, conducted detailed examinations of the damaged train coaches and railway infrastructure, gathered operational and technical records, and interviewed personnel connected to the operation and maintenance of the train.

The NSIB stated that evidence collected from witness accounts, operational logs, maintenance records, and other investigative sources would now undergo comprehensive analysis by its team of specialists.

"At the scene, investigators conducted documentation of the accident site, examined the affected rail equipment and infrastructure, gathered operational and technical information, and interviewed relevant personnel connected to the operation of the train," the bureau said.

As investigations continue, Captain Badeh assured Nigerians that the bureau would conduct a transparent, evidence-driven inquiry focused on identifying the root causes of the derailment and preventing future occurrences.

Expressing sympathy to those affected, he said, "Our thoughts and deepest sympathies are with the families who have lost loved ones, those who sustained injuries, and every passenger affected by this tragic occurrence. We understand the pain and uncertainty such an event brings, and we want the public to be assured that the Nigerian Safety Investigation Bureau has commenced a thorough and evidence-based investigation."

He further pledged that investigators would follow every lead and examine all available evidence to ensure meaningful safety improvements emerge from the probe.

"We will follow the facts wherever they lead and generate actionable safety recommendations aimed at systematically addressing identified risks and preventing a recurrence on our railway network," Badeh added.

Beyond the technical investigation, the NSIB Director-General also visited injured victims receiving treatment at healthcare facilities in the area. During the visits, he offered words of comfort and reassurance while conveying the condolences of both the federal government and the bureau to affected families.

The bureau revealed that it is collaborating closely with the Nigerian Railway Corporation, the Federal Ministry of Transportation, the Federal Ministry of Health, emergency response agencies, and relevant state authorities to ensure a comprehensive investigation.

NSIB also commended the support received from various agencies, particularly the Nigerian Railway Corporation, the Federal Ministry of Transportation, and the Nigerian Air Force, whose logistical assistance enabled rapid deployment of investigators and access to critical operational information.

Reiterating the purpose of its investigation, the bureau emphasized that its mandate is strictly safety-focused rather than punitive.

"The purpose of the investigation is not to apportion blame or determine civil or criminal liability, but to establish the circumstances of the occurrence, identify causal and contributing factors, and determine measures that can enhance the safety of rail transportation operations in Nigeria," the statement noted.

The bureau assured the public that a comprehensive report containing its findings and safety recommendations would be released upon the conclusion of the investigation. Officials believe the recommendations could play a crucial role in strengthening Nigeria's rail transport system and reducing the risk of similar incidents in the future.

The Warri-Itakpe rail corridor remains one of Nigeria's most important transportation links, connecting key economic hubs and serving thousands of passengers annually. The derailment has therefore renewed discussions about railway infrastructure maintenance, operational safety standards, and the need for continued investment in the nation's transport sector.
